The Care Partner, is responsible for engagement with patients in need of complex health care, improving outcomes for long term acute care patients. THE ROLE: Part of an exciting new initiative in primary care the Comprehensive Primary Care teams are a multidisciplinary approach to improving the health of patients in the community. The Care Partner is responsible for improving engagement and coordinating care for patients. This will include discharge care support and coordination of support services under the direction of a registered practitioner and alongside practice clinicians. Engagement will be by referral and through patient outreach to ensure best possible care and outcomes for those with complex health needs, who may be poorly engaged in planned care. This role has been introduced to improve outcomes for patient populations with complex health needs. Full and comprehensive training provided for the right candidate with ongoing professional development and supervision. KEY RESPONSIBILITIESSupporting high needs patients with long term conditions Care Coordination Health Education Reducing unnecessary hospitalisations Interdisciplinary collaboration Maintaining high level of professionalismand managing confidentiality KEY SELECTION CRITERIA: Qualifications, Skills, and Knowledge Undergraduate degree or NZQA L4 qualification, in a health-related discipline, or a qualification with a focus on health promotion, public health or similar Knowledge of Med Tech Evolution or a New Zealand PMS will be an added advantage. In-depth understanding of and proven experience within the New Zealand health system, particularly in primary, urgent or community care Ability to speak a second language would be an added advantage – Te Reo Māori, Hindi or other Pacific Language Experience working in fast-paced and high-demand environments. Flexible approach to work location and Personal Attributes A positive, Bubbly, approachable attitude that is adaptable to change and works well under pressure. Dedicate, reliable and consistent with excellent communication and follow through. A high level of integrity and confidentiality. Ability to work as a team and strong relationship building skills. A high attention to detail and accuracy. Highly motivated to improve population health outcomes Someone committed to health equity and cultural competency and can demonstrate an understanding of Tikanga Māori, Kawa Māori, and Te Tiriti o Waitangi. Other Requirements Proof of legal right to work in New Zealand (Essential). NZ Driver's license (Desirable).
