NASC Triage - Part Time (0.8 FTE / 32 hrs per week)
Needs Assessment and Service Coordination
Needs Assessment and Service Coordination provides assessment and coordination of access to a range of support services for older adults with health and disability related needs, and some younger people whose needs relate to a chronic health condition.
We are seeking a part time staff that have an understanding of working in partnership with patients and their families and the Nasc team in general.
You will be an excellent communicator, have great time management skills, are computer literate in Excel and Word as well as a record of positive relationship building.
You will have had experience in working in a health or disability related environment and a full driver's licence.

Essential
Tertiary health or disability related qualification
Current/Clean Driver's license
Understanding of disability and health needs of Older People
Ability to elicit information
Ability to be culturally appropriate with all people involved
Negotiation, advocacy and conflict resolution skills
Effective written communication and data collection
Computer literacy in Excel and Word
Understanding of budget management
Legal entitlement to work in New Zealand (permanent required)
