Tend is looking for Pharmacist Prescribers to work with us from anywhere in the country!The Pharmacist Prescriber plays an integral role as part of the wider care team model at Tend, working at top of scope to ensure effective and efficient delivery of pharmacist services to patients (including prescribing) and providing support to GPs, nurses and the wider Tend team. You will also providecare directly to people and their whānau, and support the care team to provide accessible, equitable, and optimal use and management of medication treatments. As part of your role, you will be responsible for: Medicine management and optimisation Medication reconciliation Advice, education and support Management of long-term conditions Quality, Training and Risk Equitable & culturally safe care. Our ideal Pharmacist Prescriber will have: Minimum of 5 years post-registration experience Familiarity with GP Primary Care sector & ideally, experience working in this area Experience leading and advising other health professionals or providers of care Experience in clinical quality, audit and best practice Relationship management & experience working across professional groups.
