Our client, Holistic Support Services is seeking an experienced and motivated Residential Care Officer to join their team.
Tasks include:
• Assess residents’ individual needs through observation, consultation with case managers, and review of care documentation, contributing to the planning, development, and implementation of individual support, behavioural, educational, and life-skills programs.
• Interview residents (and where appropriate, families or guardians) to understand the nature and extent of behavioural, social, emotional, or functional difficulties affecting daily living within the group housing environment.
• Implement individual care plans, structured support, training, and life-skills development programs designed to improve residents’ independence, communication abilities, and community participation.
• Supervise residents within a shared residential or group home environment to ensure safety, wellbeing, and adherence to house routines.
• Monitor residents’ behavioural, emotional, and functional progress, maintaining detailed case notes, incident reports, and progress documentation in accordance with organisational and regulatory requirements and respond appropriately to changes in condition or behaviour.
• Evaluate outcomes of implemented support programs and provide progress reports to supervisors, support coordinators, and multidisciplinary teams, recommending program adjustments where necessary.
• Refer residents to appropriate care agencies where additional professional intervention or specialised support is required.
• When required, may provide care to residents while promoting independence and dignity.
• Supervise residents within the group housing setting to ensure safety, adherence to behavioural support strategies, and compliance with organisational care standards.
• Liaise with families, allied health professionals, internal support teams and external service providers to ensure coordinated delivery of support services and continuity of care.
• Ensure compliance with workplace health and safety, duty of care obligations, and NDIS or organisational standards.
• Participate in team handovers, care reviews, and ongoing training to ensure continuity and quality of care.
Qualifications and skills:
• AQF Level Diploma or higher qualification in a relevant field
• At least one year of full time, relevant work experience within the last five years
• Demonstrated experience in implementing care plans and behaviour support plans within a residential setting.
• Ability to work independently during shifts, manage resident supervision, and respond to incidents appropriately.
• Strong communication and record keeping skills, including progress notes and incident reporting.
